The ACOFP Upper Midwest Affiliate Society works to provide the latest information on practical and technical advances in osteopathic family medical practice, continuing medical education for learning opportunities, and a wide variety of specific member services designed to for those specializing in family practice and osteopathic manipulative treatment.

Active Member Categories
These are all active member categories. Active Members shall have completed American Osteopathic Association (AOA) or Accreditation Council of Graduate Medical Education (ACGME) approved post-doctoral training in family medicine or have been in active family medicine for greater than six years. Membership in ACOFP-UMW will be valid from October 1, 2026 - September 30, 2027.

Additional Member Categories
None of the above apply to you? ACOFP-UMW has additional membership categories to connect you with our DO family physicians! Membership in ACOFP-UMW will be valid October 1, 2026 - September 30, 2027.
- The Board of Directors may extend retired membership status to active members in good standing who have permanently ceased practicing their profession due to age, long-term disability, or other qualifying circumstances.
- Associate Members work with osteopathic family physicians in this Society’s area, whether through their specialty, education, research, or other ways of supporting the field of osteopathic family medicine and the Society itself.
- Academic Members are students at osteopathic medical schools accredited by the Commission on Osteopathic College Accreditation who live in this Society’s area, study here, or are in a family medicine residency accredited by the ACGME.

Standard benefits for ACOFP-UMW include:
- Quality AOA Category 1-A continuing medical education
- Networking opportunities at the ACOFP-UMW Annual Conference and virtual events throughout the year
- Timely written communications on important issues for Osteopathic Family Physicians
- We monitor DO and Family Practice interests at state legislatures and at state agencies in rule-making and budgets; we also offer input on behalf of Osteopathic Family Physicians to state boards, committees, commissions and councils
- We collaborate with the leadership and staff of ACOFP on issues affecting Osteopathic Family Medicine Physicians
Full Conference Registration (with meals) is included in the Core and Pioneering Membership Tiers. Full Conference Registration (without meals) is included in the Student and Resident Membership Tiers. Conference Registration is not included in the Base or Retired Membership Tiers.

- Supporting the Student Chapter ACOFP at Des Moines University, including mentorship, professional development, and training opportunities for the future of the profession
- Collaborating at the National level with ACOFP to install family physicians from the upper midwest on ACOFP Boards and Committees
- Physician's Wellness Forum connects you with peers who understand the challenges of practicing medicine, this is your space to engage, reflect, and support one another.
